ELEVATED BLOOD VISCOSITY STRONGLY LINKED TO METABOLIC SYNDROME

Study suggests use of blood viscosity as additional indicator of metabolic syndrome

May 15, 2008 ? Research published in the International Journal of Cardiology demonstrates a strong relationship between elevated blood viscosity and the severity of metabolic syndrome.

Metabolic syndrome affects more than 45 million people in the U.S. The disease is a cluster of conditions? increased blood pressure, elevated insulin levels, excess body fat around the waist, elevated triglycerides, and low HDL levels ? that together sharply increase the risk of type-II diabetes and cardiovascular diseases. Metabolic syndrome is defined as having three or more of these conditions, according to National Cholesterol Education Program guidelines.

Cardiologist Lei Zhang studied 1,400 people aged 35 to 59 and placed them into one of four groups based upon their systolic blood viscosity levels. Subjects with metabolic syndrome were twice as likely to be in the highest viscosity group as the lowest. Subjects with four of the metabolic syndrome components were almost five times as likely to be in the highest blood viscosity group as the lowest.








The study authors stated that viscosity ?is strongly related to the severity of the metabolic syndrome and that the hemorheological parameters could be added to the indicators of the metabolic syndrome? as well as targeted to reduce the risk of cardiovascular diseases.

Blood viscosity has been previously associated with many cardiac risk factors including high blood pressure, LDL cholesterol, and obesity. A study published in the International Journal of Obesity noted obese patients (BMI > 28) had on average 15% higher diastolic blood viscosity than non-obese healthy controls.

Study Shows Blood Viscosity Linked to Heart Attacks and Strokes

The Blood Clinic Medical Research Report

April 23, 2008 ? Research published in the European Journal of Clinical Investigation suggests that elevated blood viscosity increases the risk for heart attacks and strokes by 342% in men with high blood pressure.

The findings are based on a study of 331 men between 40 and 64 years of age who were newly diagnosed with essential hypertension, then followed for up to 12 years. Researchers grouped patients into three categories by their diastolic whole blood viscosity (?WBV?) levels: top-tertile diastolic blood viscosity (WBV > 243), middle tertile (228 < WBV < 243), and bottom tertile (WBV < 228), (all units in millipoise [mP]). The highest blood viscosity group had more than a threefold risk of cardiovascular events than the lowest blood viscosity group (hazard ratio = 3.42, 95% confidence interval = 1.40-8.38, p-value = 0.006).

In the 331 men with high blood pressure who were studied, there were 42 first cardiovascular events, specifically, 9 subjects with myocardial infarction, 14 with unstable angina, and 19 with acute stroke.

The study authors wrote: ?Whole blood viscosity, an overall measure of flow resistance of bulk blood, depends on several factors, including cell concentration, cell aggregation, cell deformability and plasma viscosity. In our data, both WBV and hematocrit were univariate predictors of cardiovascular morbidity but only the former came out as an independent risk factor in a multivariate analysis, thus supporting the perspective that low-shear-rate (diastolic) WBV as a global marker of the whole-blood rheological properties may be a better discriminant of cardiovascular risk in hypertensive men.?

The researchers concluded that their findings highlighted the role of blood viscosity in impairing microcirculation and that a ?vicious cycle? may exist in which impaired microcirculation maintains, or even amplifies, an initial increase in blood pressure.

The report states: "This study has documented the powerful independent associations between WBV at low shear rate (diastolic)and cardiovascular events." (Ed. note. parentheses added)

Comparative evaluation of blood viscosity in diabetic retinopathy

Abstract

The viscosity of the whole blood, plasma and serum, haematocrit and plasma fibrinogen were studied in diabetic
patients with (DR) and without (D) retinopathy and in non-diabetic control subjects (C). Blood viscosity was
significantly higher in diabetics than in controls.
No significant differences in viscosity of the whole blood were found when various types of retinopathy were
compared according to the severity of retinal damage.
Plasma viscosity was significantly higher (P<0.01) than controls (C) only in diabetic patients with retinopathy
(DR). Serum viscosity was significantly increased compared with controls (C) only in diabetic
patients affected by proliferative retinopathy.
Plasma fibrinogen was significantly higher than controls (C) both in diabetics with retinopathy (DR) and
without retinopathy (D).
Haematocrit did not show a significant difference in the three groups considered (C, D, DR).

Summary. Blood viscosity (shear rates 100s -1 and
0.94s -1) and several of its major determinants
(haematocrit, plasma fibrinogen and plasma viscosity)
have been measured in 38 male insulin-treated
diabetics, aged 18-50 years, and in 38 non-diabetic
control subjects matched for age and smoking habit.
Diabetics without fundoscopic retinopathy (n= 20)
had higher mean blood viscosity than controls at the
high shear rate (7.07 cP vs 6.75 cP, p < 0.05) and the
low shear rate (21.2 cP vs 18.7 cP, p < 0.025). These
differences persisted after correction of blood viscosity
to a standard haematocrit, and were associated
with increased plasma viscosity (1.41 cP vs 1.34 cP,
p < 0.025) and plasma fibrinogen (2.9 g/L vs. 2.5 g/
L, p < 0.025). Diabetics with retinopathy (n = 18)
had higher mean blood viscosity than diabetics without
retinopathy at the high shear rate (7.53 cP vs
7.07 cP, p < 0.05) and the low shear rate (24.3 cP vs.
21.2cP, p < 0.05), associated with a higher
haematocrit (p < 0.05). Blood viscosity and
haematocrit correlated with the duration of diabetes
(r > 0.32, p < 0.05).

Increased blood viscosity in diabetics was first
described by Skovborg and co-workers [1], and has
been confirmed in some studies [2-5] but not others
[6-8]. These studies have mostly been performed on
older patients, in whom interpretation of viscosity
data is complicated by heterogeneity of the diabetes,
multiple drug therapy, and multiple pathology
including atherosclerosis, which is itself associated
with increased blood viscosity [9, 10]. Previous
studies have also ignored the effects on viscosity of
smoking [11], menstruation [12] and renal impairment
[13]. In two studies [4, 5] viscosity was related
to the presence or absence of retinopathy, but the
type of retinopathy (background or proliferative) was
not considered.
We have therefore attempted to clarify the role
of blood viscosity and its major determinants
(haematocrit, plasma fibrinogen, and plasma viscosity)
in diabetes and diabetic retinopathy, by studying
young insulin-treated male diabetics, with normal
urea and creatinine levels, who were otherwise
healthy and on no medication. Results have been
compared with a group of normal males matched for
age and smoking habit.
