Alarming H1N1 Case Fatality Rates In India

Recently released fatality rates for 2010 lab confirmed pandemic H1N1 cases in India include many states with double digits fatality rates (CFRs) among lab confirmed cases.  The largest number of deaths (261) was in Maharashtra, which had a rate of 11.3%, but Gujarat had a rate of 21.4% (194/905) while Madhya Pradesh was at 40.1%.  The highest rate was 60% (9/15) in Chhattisgarh.  The 2010 rate for the entire country was 8.4%.  In 2009 the rate was 3.7% for the entire country.

Seaweed may halt swine flu spread

Tasmanian scientists have discovered a compound occuring naturally in seaweed which could be the key to beating swine flu.

The compound is found in seaweed off Tasmania's east coast and is considered a natural defence against marine viruses and toxins.

Scientists at a private laboratory near Hobart have tested the compound against several viruses and say it profoundly inhibits the H1N1 virus.

01/14/10 - H1N1 D225G In India --- January 13, 2010 CDC has released a new series of sequences at GISAID which includes one set from India. The HA sequence had D225G, adding to the evidence for spread of the polymorphism which has been linked to case fatality rates of 100% in several countries. Although there was not information on the stats of the patient, the sample was collected in October, when the rate of fatalities in India was accelerating

01/14/10 - Caracal Cluster Pandemic Panic Likely Linked to D225G -- January 13, 2010 Olt is out of influenza vaccine, hundreds of people being vaccinated in the last two days by family physicians. General hysteria was fueled in Olt recorded influenza death last Sunday, that of women 42 years of Caracal, future in-law of the young for 21 years who died in the same case. "The woman and young man who was to be my son fell ill during the same period. None suffered from other diseases, but the evolution of influenza in November was bad from both patients, "explained Dr. Marinela Madan, Deputy Director of the Department of Public Health (DPH) River. The above translation describes the run on H1N1 pandemic vaccines in Olt County, Romania after news on the Caracal cluster spread (see map). In addition to the two fatal cases, the women's daughter had been hospitalized and her husband had had symptoms. The above translation refers to the evolution of the virus, which likely refers to the D225G/N changes in adjacent Ukraine. Although the media has referred to these changes as the "Norway mutation", the country reporting the largest number of fatal cases was Ukraine. Mill Hill released sequences on four fatalities and all had D225G, which was followed by two likely fatalities which had D225N, and the in November sequence from patients in and around Kyiv had both D225G and D225N in the same lung samples.

01/14/10 - New Ukraine H1N1 Sequences Raise Concerns -- January 13, 2010 Mill Hill has released a new series of Ukraine sequences at GISAID. The 12 Dnipropetrovsk HA sequences were from virus isolated from mammalian (MDCK) cells, while the 6 Cherkasy HA sequences were from virus grown in eggs. The samples did not have demographic data, but were isolated in early November, when most fatal cases were in western Ukraine (see map).

01/14/10 - Vaccine push follows three new swine flu deaths in Dallas-Fort Worth -- Three more H1N1-related deaths were announced Wednesday in Dallas and Tarrant counties, prompting health officials to urge more people to get vaccinated. "These deaths and the one reported last week are reminders of just how serious the H1N1 flu can be and how important it is to get vaccinated," said Dr. Sandra Parker, Tarrant County Public Health's medical director and health authority.

01/14/10 - H1N1 Re-Infections Confirmed -- We report on 3 patients with confirmed influenza A pandemic (H1N1) 2009 reinfection after successful treatment with oseltamivir. The above comments are from a report in the current EID of re-infections by H1N1 in lab confirmed cases treated with Tamiflu in Chile. This report follows media reports of other cases in the United States with at least two lab confirmed infections in individual patients. The other reports had larger gaps in disease onset dates and those re-infections are similar to many anecdotal reports.

01/05/10 - D225E and D225N H1N1 RBD Changes in Turkey -- The Refik Saydam National Public Health Agency has released 29 HA sequences from Ankara, Turkey. Several were partial sequences, but 26 covered the receptor binding domain and 8 had D225E (see list below), while one had D225N, A/Ankara/26/2009. The outcomes of these patients were not given but media reports have described a rapid in increase in H1N1 fatalities in Turkey. A large number of HA sequences with D225E has been published from Spain, and recently released GISAID sequences from Sweden and the UK also haveD225E, including three fatal cases from Sweden. The prior reports of D225G and D225N associations with fatal cases has raised concerns that changes at position 225 could alter tissue tropism or aid in immune escape, leading to more severe and fatal cases. In addition, the first reported cases of H274Y in an H1N1 patient who had not been treated with Tamiflu also had D225E as did another H274Y isolate from Tennessee, raising concerns that H274Y would pair up with receptor binding domain changes and become fixed, as happened with H274Y in seasonal H1N1.
